User Information

While provisioning any user on the Direct Provisioning Portal, personal details specific to the user must be provided in order to successfully provision them. Depending on the user type or the certificate being applied to a Direct address, the information that is required for a user to be provisioned can vary. If any user is being added to the Health Provider Directory, see the last section of this article.

Please note that any fields not listed in the sections below are optional.

Admin (No Certificate)

Even without a certificate an Admin user must provide some personal information in order to be provisioned successfully. This includes the following:

  • Notification Types (only 1 required)
  • Direct Messaging Address
    • Note: Without a certificate, this can be any correctly formatted email address. It is advised to simply enter the user’s Notification Email in this field as a result.
  • Notification Email
  • First Name
  • Last Name

Direct User and Admin (Organization Certificate)

These are the simplest users to provision in terms of required information, with the only required fields being as follows:

  • Notification Types (only 1 required) (Admin Only)
  • Direct Messaging Address
  • Notification Email
  • First Name
  • Last Name

Direct User and Admin (Address Certificate)

Provisioning a user covered by an Address Certificate requires significantly more information as well as a completed Declaration of Identity Document (See here for details on filling out this document) in order to successfully provision. The following fields are required for these users:

  • Notification Types (only 1 required) (Admin Only)
  • Direct Messaging Address
  • Notification Email
  • First Name
  • Last Name
  • Address
  • City
  • Postal Code
  • State
  • Country
  • Photo ID Type
  • State (depends on Photo ID Type chosen, will not be present for Passport option)
  • Expiration Date
  • A completed Declaration of Identity Document

Trusted Agent

It does not matter whether or not a certificate is applied to a Trusted Agent, the information required for this user type is always the same. A completed Declaration of Identity Document (See here for details on filling out this document) will always be required as well. The required fields are as follows:

  • Notification Types (only 1 required) (Admin Only)
  • Direct Messaging Address
    • Note: If no certificate is being applied, then this field can be any correctly formatted email address. It is advised to simply enter the user’s Notification Email in this field as a result.
  • Notification Email
  • First Name
  • Last Name
  • Address
  • City
  • Postal Code
  • State
  • Country
  • Photo ID Type
  • State (depends on Photo ID Type chosen, will not be present for Passport option)
  • Expiration Date
  • A completed Declaration of Identity Document

Users to be Included in the Provider Directory

For any users who must be added to the Health Provider Directory, the fields required will be expanded. The required fields for these users (in addition to the required fields from the above sections) will include the following:

  • Provider Specialty
  • Provider Role
  • Use Tenant Org Info
    • Note: If this checkbox is selected the user will be entered into the Health Provider Directory with the same Address and Phone information as the entire tenant. If this is not desired do not click the checkbox. However in this case additional fields will be required which include:
      • Organization Name (For This User)
      • Organization Address (For This User)
      • Organization City (For This User)
      • Organization State (For This User)
      • Organization Postal Code (For This User)
      • Organization Phone (For This User)
